Every year, thousands of industrial workplace accidents worldwide result from the incorrect or inadequate isolation of energy sources. The unexpected energizing of machinery or equipment during maintenance, repair, or cleaning can lead to severe consequences such as electric shock, crushing, burns, and explosions. The LOTO (Lockout/Tagout) system is the most effective energy isolation method developed to eliminate these risks.

What Is LOTO?

LOTO is a procedural safety system applied to safely isolate and lock hazardous energy sources (electrical, pneumatic, hydraulic, chemical, thermal, mechanical, etc.) during maintenance or servicing work.

Lockout: Preventing equipment from being re-energized by attaching a physical lock to the energy isolation point (fuse, valve, breaker, etc.).

Tagout: Hanging a warning tag on the isolation point when lockout cannot be performed, or in addition to lockout.

Why Is LOTO Mandatory?

International occupational safety regulations explicitly mandate the implementation of LOTO:

  • OSHA 29 CFR 1910.147 (USA): The “Control of Hazardous Energy” standard regulates LOTO procedures in detail.
  • ILO Code 28: An international guideline on energy isolation during machinery maintenance and repair.
  • Turkey — Occupational Health and Safety Regulations: The requirement for energy isolation is included within the Regulation on Health and Safety Conditions for the Use of Work Equipment.

LOTO Procedure — Step by Step

A proper LOTO implementation should include the following steps:

1. Preparation
All energy sources of the equipment (primary and secondary) are identified. The necessary LOTO equipment (lock, tag, hasp, etc.) is selected.

2. Shutting Down the Equipment
The equipment is shut down using the normal procedure. It is verified that all moving parts have come to a stop.

3. Energy Isolation
All energy isolation points (fuse, switch, valve, etc.) are closed or disabled.

4. Lockout
A physical lock is attached to each isolation point. Each worker attaches their own lock — no one else can remove it.

5. Tagout
A “Work in Progress — Do Not Open” warning tag is hung next to the locks. The tag must include the worker’s name, date, and contact information.

6. Releasing Residual Energy
Capacitors are discharged, pressurized systems are vented, springs are released, and raised equipment is supported/blocked.

7. Verifying Isolation
Before starting work, it is tested and confirmed that the equipment is de-energized (using a voltmeter, pressure gauge, etc.).

8. Completing Work and Removing Locks
After the work is completed, each worker removes only their own lock. The equipment is safely brought back into service.

LOTO Equipment — What Does It Do?

Safety Padlocks
The core component of the LOTO system. Plastic-bodied models provide electrical insulation, while steel models offer high strength. Each should be unique to an individual worker, with a different key.

Electrical Fuse Locks (MCB Lockout)
Prevent miniature circuit breakers (MCBs) from being switched on unintentionally. Models are available for both outward-opening and inward-opening breaker types. The correct model should be selected based on MCB size (≤12mm, ≤20mm, etc.).

Circuit Breaker Locks
Used to lock large industrial switches and circuit breakers. Their clamp-based designs allow them to fit switches of various sizes.

Valve Locks
Lock valves on gas, liquid, or steam lines. Models are available specifically for different valve types such as gate valves, ball valves, and butterfly valves.

Pneumatic Lockout Equipment
Used to lock connection points and air valves in pressurized air systems.

Lockout Hasps
Used when more than one person needs to work on the same isolation point. Models are available offering 6 to 12 lock openings.

Group Lockout Boxes
In complex maintenance work involving multiple isolation points, all keys are placed inside a group lockout box, and the box itself is locked. Each worker attaches their own lock to the box.

Lockout Tags
The informational element that completes the lock. Made of durable plastic or paper material; contains warnings such as “Do Not Open” or “Work in Progress.”

Common LOTO Mistakes

  • Using a shared lock instead of a personal lock
  • Failing to isolate all energy sources (overlooking secondary sources)
  • Failing to release residual energy (capacitors, pressurized air)
  • Considering a tag sufficient without attaching a physical lock
  • Failing to put the LOTO procedure in writing
